If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill so you do not require a doctor’s sickness certificate.
Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work, this is called self-certification. You can complete a self-certification form (SC2) which may be available from your employer and is on the HMRC website.

Fit Notes and GP Assessment
A fit note is issued following an assessment by a GP to determine your fitness for work. This assessment may take place via a telephone consultation or an in-person appointment, depending on your circumstances. The duration of a fit note will be based on the nature and progression of your condition.
To request a fit note, please contact the surgery between 08:30–10:30 or 14:30–18:30, Monday to Friday, to speak with a GP about your symptoms and how they are affecting your ability to work.
Please note that Mondays and Fridays are our busiest days, so if your enquiry is not urgent, you may experience shorter wait times on other weekdays.
