If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill so you do not require a doctor’s sickness certificate.
Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work, this is called self-certification. You can complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

If you’ve been off sick for more than 7 days
To request a new fit note:
A fit note is based on an assessment by a GP about your fitness for work. In order to complete a fit note a GP will need to assess you , either through telephone consultation or a face-to-face appointment. The length of a fit note will depend on the ongoing nature of your condition.
Please call the surgery between the hours of 08:30 -10:30 or 14:30-18:30 Monday to Friday to speak with a GP

Continuation fit notes:
If you have a fit note that is coming to an end you can request a continuation fit note using one of the following options
- Online fit note request please select “I have an admin query” then “Fit (sick) note” and complete the questionnaire
- You can also visit the surgery to complete a paper form to request a continuation fit note
Fit note requests are not an emergency. Please allow 5 working days for the doctor to respond to your fit note request; notes can be back-dated for up to 2 weeks after they have been requested.
Sometimes, the doctor requires more information about the ongoing nature of your condition, and may decline your request until they have sufficient information
